Please be aware that no student will be permitted to begin classes without the proper health records on file in the Administration Office. Immunizations, health exams and screenings, as well as the appropriate documents, may be obtained from your child’s physician or the Broward County Health Department. NEW students must have the original, signed DH 3040 Health Exam on file. ALL students’ immunization information must be current and documented on an original DH 680 Florida Certification of Immunization form, including the exact day/month/year for each immunization, and physician/authorized signature with expiration date. Please note: the Immunization Record must remain current throughout the school year. |
