Immunization Requirements For School Entry Immunization Requirements For School Entry

Please be aware that no student will be permitted to begin classes without the proper health records on file in the Administration Office. Immunizations, health exams and screenings, as well as the appropriate documents, may be obtained from your child’s physician or the Broward County Health Department. NEW students must have the original, signed DH 3040 Health Exam on file.

ALL students’ immunization information must be current and documented on an original DH 680 Florida Certification of Immunization form, including the exact day/month/year for each immunization, and physician/authorized signature with expiration date. Please note: the Immunization Record must remain current throughout the school year.

Immunizations Required for K4 Entry

Diphtheria-Tetanus-Pertussis Series
Haemophilus influenzae type b (Hib)
Hepatitis B
Measles-Mumps-Rubella (MMR)
Polio Series
Varicella (Chicken Pox)

Immunizations Required for K5 Entry

Diphtheria-Tetanus-Pertussis Series
Hepatitis B Series
Measles-Mumps-Rubella (two doses of Measles vaccine, preferably MMR)
Polio Series
Varicella (Chicken Pox)

Immunizations Required for 7th Grade Entry

Hepatitis B Series
Second Dose of Measles vaccine (preferably MMR)
Tetanus- Diphtheria Booster
It is recommended that students receive a scoliosis screening at this time.
